Sounds beastly. Almost wish it was always that loud, but I'm wondering if somehow the car is running too rich on those rare occasions.

Check valvetronic. Mine had this too and shaky idle, due to valvetronic sensor not working the car goes rich on idle but it wont pop until you drive it. Its hard to notice.
Do you have fuel smell on idle and when on light cruise?
